29 CFR 1910.178(l)(4)(iii): The employer did not ensure that an evaluation of each powered industrial truck operator's performance was conducted at least once every three years: a/ SMP Inc./Phillipsburg NJ Site - Bay Area: Forklift Operators were not re-evaluated at least once every three years after the initial training. Evaluation to include, but not be limited to, driving skills, review of safe operations, etc. The evaluation should be documented to include the name of the operator evaluated, the name of the person(s) providing the evaluation and the date of the evaluation. Observed on/or about 12/20/2013.
