29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a) Jobsite, 811 East Carson Street, Pittsburgh, PA:  On or about August 9, 2013, a metal quad box was lying on the floor being used to power office equipment and was not being used in accordance with the listing and labeling instructions.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a) Jobsite, 811 East Carson Street, Pittsburgh, PA:  On or about August 9, 2013, the electric cord on the Dayton fan # 400100, was not provided with adequate strain relief at its attachment plug.    b) Jobsite, 811 East Carson Street, Pittsburgh, PA:  On or about August 9, 2013, the electric cord on the metal quad box was not provided with adequate strain relief at its attachment plug.
