29 CFR 1910.37(a)(4): Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) must be in proper working order at all times.  a) Production area South wall- On or about July 17, 2013, an emergency light would not illuminate when the test button was pushed, which would not provide the required illumination for employee egress in the event of emergency power loss.  b) Production area East wall- On or about July 17, 2013, an emergency light would not illuminate when the test button was pushed, which would not provide the required illumination for employee to egress in the event of power loss.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):  a) Switch gear room - On or about July 17, 2013 employees were working on a 277 VAC motor control center with exposed, unprotected live parts that had not had the appropriate lockout devices or tagout devices affixed to isolate the electrical power and to prevent unexpected energization, start up or release of the stored energy as a control measure to prevent injury to the employees.
