29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about August 9, 2013, Southwest Dekalb High school addition 2863 Kelley Chapel Road, Decatur, Georgia: Employees operating tools including but not limited to Bosch 120 volt, 15 amp chipping hammers and Skillsaw 120 volt, 15 amp saws using extension cords plugged into the GFCI temporary power supply were exposed to electric shock and electrocution hazards when the GFCI temporary power supply installed on the site did not operate correctly because the ground was not attached to the GFCI.
