29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about August 12, 2013, at 2800 Trophy Club Rd., Trophy Club, Texas, employees installing 8 inch sewage PVC pipes inside a trench measuring approximately 18 feet deep, 5 feet wide and 100 feet long were not provided a means of egress. Employees were exposed to cave-in hazards.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about August 12, 2013, at 2800 Trophy Club Rd., Trophy Club, Texas, employees installing 8 inch sewage PVC pipes inside a trench were not protected from falling rock/dirt when the spoil materials were located along the edge of both sides of the trench. Employees were exposed to struck by and cave-in hazards.
