On or about August 12, 2013, at 2800 Trophy Club Rd., Trophy Club, Texas, employees installing 8 inch sewage PVC pipes inside a trench measuring approximately 18 feet deep, 5 feet wide and 100 feet long were not provided a means of egress. Employees were exposed to cave-in hazards.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about August 12, 2013, at 2800 Trophy Club Rd., Trophy Club, Texas, employees installing 8 inch sewage PVC pipes inside a trench were not protected from falling rock/dirt when the spoil materials were located along the edge of both sides of the trench. Employees were exposed to struck by and cave-in hazards.
