29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about August 9, 2013, at 2863 Kelley Chappel Road, Decatur, Georgia: Employees installing stone and guard rails while using corded tools with extension cords were exposed to electric shock and electrocution hazards when the temporary GFCI power installed on the site was not grounded.
