(a) 588 Plainview Road, Plainview, NY (Worksite), aerial work platform - Employees operated the newly acquired 2012 Ford F-550 commercial truck, VIN # 1FDUF5GY7CEB79794, equipped with a Terex Hi-Ranger TL-37 Aerial Work Platform, Serial # 2120947135, and were not trained in the safety-related work practices associated with operating the vehicle and auxiliary equipment on, or about August 8, 2013. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

(a) 588 Plainview Road, Plainview, NY (Worksite), aerial work platform - Employees operated the newly acquired 2012 Ford F-550 commercial truck, VIN # 1FDUF5GY7CEB79794, equipped with a Terex Hi-Ranger TL-37 Aerial Work Platform, Serial # 2120947135, without deploying the outriggers on, or about August 8, 2013. Note: Abatement for this violation was previously documented; therefore, the employer is not required to submit abatement certification for this violation in accordance with 29 CFR 1903.19.
