29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. Site - On or about August 6, 2013, employees were not utilizing fall protection while performing roof work. Ken Stanley d/b/a A+ Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501 (b)(11) , which was contained in OSHA inspection number 399082, citation number 2, item number 1 and was affirmed as a final order on 10/31/2012 , with respect to a workplace located at Eastern Slope Inn Resort 2760 White Mountain Hwy. North Conway, NH. Note: This case went into debt collection on February 15, 2013. A+ Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926. 501 (b)(11), which was contained in OSHA inspection number 315790758, citation number 1, item number 2, issued on 9/30/2011 and was affirmed as a final order on 11/4/2011, with respect to a workplace located at Main Street and Winter Street. Haverhill, MA. Note: This case went into debt collection on December 5, 2011.
