A+ Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926. 501 (b)(11), which was contained in OSHA inspection number 315790758, citation number 1, item number 2, issued on 9/30/2011 and was affirmed as a final order on 11/4/2011, with respect to a workplace located at Main Street and Winter Street. Haverhill, MA. Note: This case went into debt collection on December 5, 2011.
