Job site located at 703 W 24th Street, Bellevue, NE 68005-Employees working on a roof were exposed to a fall hazard of 10 feet to the ground level and had not received any fall protection training from the employer.

Job site located at 703 W 24th Street, Bellevue, NE 68005-The ladder being used to access the working area of the roof did not have the side rails extrended at least 3 feet above the landing surface of the roof exposing the employees using the ladder to a fall hazard of 10 feet to the ground level.
