29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Worksite - The Employer did not establish a Lockout/Tagout Program for employees who work on/unjam power equipment such as, but not limited to John Deere 220 E-Cut walk Greens Mowers, serial # 1M0220EAVDM040636, serial # 1M022EAADM040587, and serial # 1M0220EAVDM040586; on or about 08/12/13.  Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
