The employer failed to install permanent wiring in the following locations:   a. Rotocure Line 1: A 480 volt electrical cord that was connected to the Web & Design Blowers, ran along an adjacent aisle, over and under equipment and terminated at a power panel located in the vicinity of the Urethane Coating Room.   b. Urethane Room #2: An electrical cord that was connected to an alarm inside the urethane room, was run out of the room, over an adjacent aisle, and entered a side vent of an electrical panel.  c. Urethane Room #2: An extension cord that was plugged into a receptacle inside the Urethane Room, was run out of the room, taped to a wall and floor, and then entered a vent on the side of an electrical panel to power an electrical relay.
