29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:      a) On or about 8/06/13 in apartment 9 of building under renovation at 786 Kenmore Ave., Buffalo, NY:  A Marley pedestal fan was used to provide ventilation. The Fan was connected to 120-volt ac power via an extension cord. The plug on the cord of this Fan lacked a grounding prong.         NO ABATEMENT CERTIFICATION REQUIRED
