29 CFR 1910.272(m)(4):     The employer did not implement procedures in the grain handling facility for the use of tags and locks which would prevent the inadvertent application of energy or motion to equipment being repaired, serviced, or adjusted, which could result in employee injury:    Employees who performed servicing and maintenance on drag conveyors and bucket elevators in preparation for harvest were exposed to the unexpected energization of the equipment and energy control procedures had not been developed, documented, or utilized to ensure the energy is controlled during this and other service and maintenance operations at the facility.

29 CFR 1910.305(g)(2)(iii):     Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    Employees were exposed to the hazard of electrical shock when using the sweep auger in Bin 34, because the flexible power cord was not provided strain relief where it entered the control panel with devices and fittings that would have prevented tension from being transmitted to the conductors.
