Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish to each of his employees employment and a place of employment which are free from recognized fir and explosion hazards that are causing or are likely to cause death or serious physical harm to his employees: This violation most recently occurred on or about August 10, 2013, where the employer, by not ensure that a 30-inch crude oil transportation pipeline was emptied, drained and thoroughly cleaned before welding started, exposed employees to fire and explosion hazards.
