29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a)  Mid-America Plating LLC, at 4877 National Western Drive #309, Denver, CO: On and preceding 8/20/13, Mid-America Plating LLC did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from acids or caustic liquids.  One employee worked on a chromate conversion coating operation.  The employee placed parts into and from tanks containing solutions of Haviland Products Company ND-68 G (containing sodium hydroxide), Haviland Products Company Prodigy Skybrite 100 (containing 5% nitric acid), and Haviland Products Company Havacoat AL-Y (containing 95% chromic acid).  Each of these is a corrosive liquid.  The employee performing the tasks wore prescription eyeglasses.  The employer did not ensure that the employee wore appropriate eye protection.  This condition exposed the employee to a corrosive liquid hazard.
