While the cover was not in place, the floor hole was not constantly attended by someone or protected by a removable standard railing:  This violation occurred on July 30, 2013, at an establishment located at 715 Skyline Drive, Duncanville, Texas; at the water treatment area where employees were exposed to:     1)     A floor drain approximately 10 x 12 inches; 2)     A floor drain approximately 4 inches in diameter.
