29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms, and service rooms are not kept clean and orderly and in a sanitary condition:           (a)  Globus Printing and Packaging Company, Minister, Ohio:  On or about August 13, 2013, the employer did not ensure that work areas were kept from accumulations of paper dust.  The floor, process equipment, building purlins and girts, and other horizontal surfaces in the baler room had several inches accumulation of dust.  The dust was analyzed and determined to be explosive.

29 CFR 1910.106(e)(6)(ii):  Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 F, were dispensed into containers without the nozzle and container electrically interconnected.           (a)  Globus Printing and Packaging Company, Minister, Ohio:  On or about August 13, 2013, the employer did not ensure that when employees dispensed a Category I flammable liquid that the nozzle and container were grounded.  Dispensing a flammable liquid without the containers grounded can generate an electrical spark and ignite the liquid. Employees routinely dispensed 2-propanol, a Category I flammable liquid and MRC Roll Cleaner, a Category II flammable liquid, into containers and were exposed to fire hazards from the flammable liquid.

The employer did not evaluate the hazards associated with flammable liquids which were used in the printing department and corrosive acid in the fork truck batteries.  As a result, employees were not provided with appropriate personal protective equipment to protect their eyes, face, and body from the hazards associated with these materials.
