29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment shall contain an equipment grounding conductor. a. At the electrical receptacle on the pole adjacent to Laser Cutter #7 where an employee operated the machine, a ground was missing from a portable fan, thereby exposing the employee to a electrical contact hazard. b. At the worktable adjacent to Laser Cutter #7 where an employee used an angle grinder to sand burrs from metal, the ground prong was missing from an extension cord, thereby exposing the employee to an electrical contact hazard.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a. At the ADIGE Lasertube LT8 laser cutter machine where employees cut round, square, and rectangular metal tube, there was an inoperable interlock on the personnel access door adjacent to the operators work station, thereby exposing employees to caught-in and other hazards. The Laserflex Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 314585498, citation number 1, item number 1 and was affirmed as a final order on August 2, 2010, with respect to a workplace located at 3649 Parkway Lane, Hilliard Ohio, 43026.
