29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  On or about 08/08/2013 at 150 Pearl Nix Parkway Gainesville, GA; employees performing bricklaying operations were exposed to struck-by hazards from approximately 32' to the lower level where there were no toeboards installed.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:   On or about 08/08/2013 at 150 Pearl Nix Parkway Gainesville, GA; employees performing bricklaying operations were exposed to 30' falls to the lower level where the working level was not fully decked.  Ordner Consrtuction Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number312977929, citation number 01 item number 001(a) and was affirmed as a final order on 04/26/2010, with respect to a workplace located at 4413 Birkland Place Easton, PA.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On or about 08/08/2013 at 150 Pearl Nix Parkway Gainesville, GA; employees performing bricklaying operations were exposed to a 32' fall to the lower level where the ladder did not extend the full height of the scaffold to the working level and employees were observed scaling the scaffold frame beyond the reach of the ladder.   Ordner Consrtuction Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number312977929, citation number 01 item number 002 and was affirmed as a final order on 04/26/2010, with respect to a workplace located at 4413 Birkland Place Easton, PA.
