29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  On 14 August 2013, two workers using pneumatic nail guns to install roof trusses on a residence under construction were not wearing eye protection, exposing the workers to a struck by hazard from flying debris or objects to the face or eyes.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On 14 August 2013, two workers were installing roof trusses on the residence under construction that was one story in height without fall protection, exposing the workers to a fall hazard of approximately 8 feet 5 inches and more to the ground below.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  On 14 August 2013, two workers accessed the top plate of a one story residence under construction by using the horizontal bracing between the studs as a wooden job-made ladder, where the spacing between the braces was approximately 25 inches apart, exposing the workers to a fall hazard of approximately 8 feet 5 inches to the ground.
