On 14 August 2013, a worker was operating a masonry saw to cut brick while a helper was spraying water on the surface, both without eye protection, exposing the workers to the hazard of being struck by flying debris or objects to the face and eyes.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  On 14 August 2013, workers were using a masonry saw that was plugged into a 120 volt electrical extension cord which had strain relief damage at the plug, exposing the workers to an electrical hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Support legs were used as a means of access.  On 14 August 2013, mason workers were accessing a one-tier tubular-welded frame scaffold to perform brick laying acivities in which a ladder was not provided to access the scaffold, exposing the workers to a fall hazard from approximately 7 feet and 3 inches.
