29 CFR 1910.303(b)(6): 29 CFR 1910.303(b)(6): Equipment located in damp or wet locations and exposed to a corrosive salt atmosphere was not identified for that purpose:    a. Electrical circuit breaker/fuse boxes, not designed for wet processes, were subjected to rain water; during rain, employees operated circuit breaker boxes including when standing on wet floors.    b. Electrical lighting fixtures (110 V) were exposed to rainwater; some of the fixtures have failed because of the conditions and have not been taken out of service.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    a. In the back room, 110 V electrical equipment was plugged into a multi-outlet adapter with missing ground pins.    b. In the area behind the counter, 110 V electrical equipment was plugged into a duplex outlet with an open ground.    (Note: equipment was periodically used by employees standing on a wet floor.)    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. Openings existed in the fuse box and a circuit breaker box in the back room; employees powered equipment by engaging switches in the boxes (periodically while on wet floors). 29 CFR 1910.22(a): 29 CFR 1910.22(a)(1):  Place(s) of employment were not kept clean and orderly, or in a sanitary condition:  a. Employees working at the business were exposed to wet conditions because of roof leaks; although previously reported the leaks have not been repaired to prevent the continuing presence of rain leaking through the ceiling, exposing electrical fixtures to wet conditions, and requiring employees to walk and work on wet floors.  The condition existed throughout the building, including the presence of standing water in the basement.   Abatement certification and abatement documentation is required for this violation.
