All hand and power tools and similar equipment, whether furnished by the employer or the employee, were not maintained in a safe condition.   a)  Worksite, 56 Muriel St. Lawrence NY - Employees were using a Dewalt Drill. model #DW130V, serial #178239, to mix stucco.  The outer insulation on the cord cut, exposing the interior insulation; on or about 08/14/13.                 Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a)  Worksite, 56 Muriel St. Lawrence NY - Employees were applying stucco to a two story residential home while working from a fabricated frame scaffold that was not fully planked.  The scaffold had two planks installed at the working level approximately 12ft above the ground; on or about 08/14/13.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
