29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 14, 2013, and at times prior thereto. employees were working on a steep roof and were not provided with fall protection. Employees were exposed to a fall of at least 10 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about August 14, 2013, and at times prior thereto, the ladder employees used to access the roof of the commercial did not extend at least 3 feet above the edge of the roof. Employees were exposed to a fal of at least 10 feet.
