(a) On or about 08/01/2013, at establishment, for employees welding on stainless steel. Employer provided OSHA with only personal air monitoring results for hexavalent chromium exposure from a 06/16/2011 monitoring event. Data indicated that exposure to two welders was not obtained due to two pump failures.
