29 CFR 1910.67(c) (2) (v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:              This violation occurred on or about August 13, 2013, at a jobsite located at 6300 Samuel Boulevard in Dallas, Texas 75228, where at least one (1) employee on the northeast side of a parking lot was repairing a damaged advertisement sign from an aerial lift without the use of a body belt and lanyard.
