29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a) At Alliance Water Resources, Inc. on or about August 14, 2013 the railing along the edge of the second floor had two locations which did not have a mid rail as required by 1910.23(e)(3)(v).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) In the Scrubber Room at Alliance Water Resources, Inc. on or about August 14, 2013, employees exposed hydrochloric acid 20 deg. corrosive liquid while transfering the acid from a 55 gallon drum to a bucket and then using the acid to remove scale were not provided a shower and eye wash in the work area for immediate use.

29 CFR 1910.165(d)(2): Tests of the reliability and adequacy of nonsupervised employee alarm systems were not made at least every two months: (a) At Alliance Water Resources, Inc. the Regal Series 3000 Chlorine Alarm had not been tested and verified as working for over one calendar year. Alliance Water Resources, Inc. had not established and implemented a procedure for testing the chlorine alarm.
