29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Southeast side of building - employees were exposed to a 7'-7" fall hazard, in that, the ladder side rails extended 33" above the roof landing. Violation observed on or about 8/14/13.  Done Rite Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.10531(b)(1), which was contained in OSHA inspection number 899046, citation number 1, item number 2, and was affirmed as final order on 05/28/13, with respect to a workplace located on 304 Buckingham Place, Palm Harbor, FL 34684.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. At the site - the employer did not develop a written training certification for employees exposed a 7'-7" fall hazard while working on a 4 in 12 pitched roof. Violation observed on or about 08/14/13.
