29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate: Upper storage floor, southeast corner of warehouse. On or about August 14, 2013, employees were exposed to a struck-by hazard of the upper storage floor collapsing from the weight of significantly heavy items such as, but not limited to, lengths of industrial strength steam and hot water flexible tubing, while they worked underneath retrieving tools, and performing administrative duties.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour: North wall of warehouse. On or about August 14, 2013, employees were exposed to burns from fire and explosion, as cylinders of oxygen were stored directly adjacent to cylinders of acetylene, in the warehouse where they moved supplies and equipment.
