29 CFR 1926.102(a)(1):     Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On 8/02/2013, employees from AZ Contracting LLC were working on a roof of a residential building located at 206 Morning, St. Peters, MO.  Two employees were not wearing eye protection while using pneumatic nailers.

29 CFR 1926.501(b)(13):   "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On 8/02/2013, employees from AZ Contracting LLC were working on a roof of a residential building located at 206 Morning, St. Peters, MO.  Five employees and the crew leader, Estevan Guzman, were working on the roof of the residence, which was approximately 16 feet above the ground, without the use of guardrail systems, safety net systems, or personal fall arrest systems

29 CFR 1926.1053(b)(1):    When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     On 8/02/2013, employees from AZ Contracting LLC were working on a roof of a residential building located at 206 Morning, St. Peters, MO.  The employees were using a ladder improperly.  The ladder was not extended to three feet beyond the landing or edge of the roof.

29 CFR 1926.1053(b)(16):   Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not  withdrawn from service until repaired.   On 8/02/2013, employees from AZ Contracting LLC were working on a roof of a residential building located at 206 Morning, St. Peters, MO.  Six employees were using a ladder to access a roof, which was approximately 8 feet above the ground.  The  defective or damaged portable ladder, no serial number, had damaged , broken or bent rungs.
