29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance. (a) Procedures were not developed, documented and utilized for the control of hazardous energy such as but not limited to; the electrical disconnect, the gravity and the movement of the flywheel on equipment such as but not limited to the Kurimoto C2F-2500W mechanical forging press located on lines such as but not limited to Line 4. Employees performing servicing such as changing dies in equipment such as but not limited to; the C2F-2500W mechanical forging press located on lines such as but not limited to; Line 4 where employees were utilizing a safety interlock plug as isolation by removing the plug for control of each hazardous energy. The TFO Tech Co., Ltd was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 507958, citation number 01, item number 002 and was affirmed as a final order on October 10, 2013, with respect to a workplace located at 221 State St., Jeffersonville, OH 43128.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. (a) Each employee performing servicing such as but not limited to changing dies on equipment such as but not limited to the Kurimoto C2F-2500W mechanical forging press were not trained on the method and means of shutdown and locking out the forging presses. Employees were trained to use the removal of a safety plug as lockout. The TFO Tech Co., Ltd was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 507958, citation number 1, item number 5 and was affirmed as a final order on October 10, 2012, with respect to a workplace located at 221 State St., Jeffersonville, OH 43128.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. (a) TFO Tech did not ensure that each employees (temporary) operating powered industrial trucks at their location had completed training consisting of formal instruction and practicial training in that temporary workers were receiving their classroom training in the form of a generic test done by Remedy Intellegent Staffing. The practical training being done by TFO employees was not adequate in that the evaluations dates of the evaluations were missing on some of the practical forklift evaluations, one evaluation was not filled out and checkmarks were being done consistently for the yes box on the evaluation regardless of the questions. The TFO Tech Co., Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(2)(ii), which was contained in OSHA inspection number 507958, citation number 01, item number 007 and was affirmed as a final order on 10/10/12, with respect to a workplace located at 221 State St., Jeffersonville, OH 43128.
