29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized. The specific safety-related work practices shall be consistent with the nature and extent of the assoicated electrical hazards: (a) In the MCC (Motor Control Center) Switch Gear Room where work was performed, safety-related work practices were not outlined or identified to be used for employees working on equipment or circuits which were energized causing Arc-Flash injuries as observed on or about August 16, 2013.

While working on the Master Clad 15KV Switch Gear in the Motor Control Center (MCC) Switch Room, the equipment was not disconnected from its electrical energy source causing injuries to the employee as observed on or about August 16, 2013.

29 CFR 1910.333(b)(2)(iv): Verification of deenergized condition. The requirements of this paragraph shall be met before any circuits or equipment can be considered and worked as deenergized. (a) In the Motor Control Center (MCC) Switch Room, where preventative maintenance was being performed on high voltage switch gear where 13Kv was present, verification of the deenergized condition was not established exposing the employees to the hazard of electrical shock and arc-flash explosion as observed on or about August 16, 2013.
