29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation:     On or about August 15, 2013, and at times prior thereto,  machine guarding was not provided to protect employees from point of operation on the following press brakes:   a)  Amada HDS 1303NT,   b)  Amada Model RG-80,   c)  Amada HDS 1303NT.     Employees were exposed to the hazard of amputations.
