29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: Portland Air National Guard Keyhole project: Employees were using a MQ Power generator at single phase 240/120 volt without ground fault protection. This exposed employees to potential shock and burn hazards.

29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did not use employee signaled that it is safe to do so. Portland Air National Guard Keyhole project: The Volvo A25C Dump Truck was in operation and did not have an operational back-up alarm. This exposed employees to a potential struck-by hazard from working in the area of the equipment with no alarm.
