Overhead cranes were not inspected for defects on a daily basis for items including but not limited to; all functional operating mechanisms for maladjustment, deterioration or leakage of air or hydraulic systems.  (a) On or about  August 15, 2013 at 11021 N. POMONA AVENUE Kansas City Missouri 64153 , employees in the machine shop, manufacturing  and production area were exposed to a struck by hazard from possible failure of overhead cranes because monthly inspections for abnormalities were not accomplished or documented.

(a) On or about August 15, 2013, at 11021 N. Pomona Kansas City Missouri, 64089, employees in the manufacturing shop area were exposed to a hazard from a grinder wheel. The wheel was worn to the point there was a one and a half inch  gap between the grinder wheel and the attached guard.

29 CFR 1910.305(g)(2)(iii):   Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.   (a) On or about August 15, 2013, at 11021 N. Pomona Kansas City Missouri, 64089. ThePBI Dansensor machine did not have adequate strain relief at the plug-in end there was not adequate insulation as the insulation had pulled back exposing interior wiring.
