29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.   The employer does not ensure each platform unit is installed so that the space between adjacent units is no more than 1 inch. This violation was observed on or about August 9, 2013, on the exterior west side of the single family dwelling where employees were exposed to an approximately 6 ft 8 inch fall hazard when performing masonry work from fabricated frame scaffolding without the gap between planks being less than one inch.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform unit is installed so that the space between adjacent units and the space between the platform and uprights are no more than 1 inch wide.

29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):    The employer does not ensure each end of a platform, not cleated does not extend over the centerline of its support at least 6 inches. This violation was observed on or about August 9, 2013, on the exterior west side of the single family dwelling where employees were exposed to an approximately 6 ft 8 inch fall hazard when performing masonry work from fabricated frame scaffolding without the planking being cleated or extended over end at least 6 inches.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform , not cleated or otherwise restrained by hooks or equivalent means, extend over the centerline of its support by at least 6 inches.

This violation was observed on or about August 9, 2013, on the exterior west side of the single family dwelling where employees were exposed to an approximately 6 ft 8 inch fall hazard when performing masonry work from fabricated frame scaffolding without base plates installed. Crossbraces were used as a means of access.    The employer does not ensure each scaffold platform 2 feet or more are provided with access. This violation was observed on or about August 9, 2013, on the exterior west side of the single family dwelling where employees were exposed to an approximately 6 ft 8 inch fall hazard when performing masonry work from fabricated frame scaffolding without a point of access or other type of ladder.
