29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: On or about August 8, 2013, at the above address jobiste, an employee was exposed to a 7 foot fall hazard from a scaffold platform that had no ladder access. PLEASE NOTE: AN ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity): On or about August 8, 2013, at the above address jobsite, a worker was exposed to a 7 foot fall hazard while working from a scaffold with missing guard rails at open sides and ends of the scaffold. PLEASE NOTE: AN ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION.
