29 CFR 1910.132(a): Protective equipment was not used or provided when necessary whenever hazards capable of causing injury and impairment were encountered:    On or about August 13, 2013, and times prior thereto, at 302 South Industrial Drive in Blytheville, Arkansas, in the shop area,    (a)  Employees operating the Toyota forklift did not wear the seat belt provided.      (a)  The Kalmar Gray and Yellow forklifts used to transport parts throughout the shop area were not outfitted with a seat belt.      These conditions exposed employees to the hazard of being thrown, ejected and crushed in the event of a tip-over.

This condition exposed employees to struck-by and caught-in hazards if the machines and/or equipment were unexpectedly energized.

This condition exposed employees to contact with the cutting blade.    (b)  In the machining area, a guard was not furnished over the rotating chuck on the Moriseiki lathe.  This condition exposed employees to the hazard of contact with rotating parts.
