29 CFR 1926.451(b)(4):   Each end of a platform, which was not cleated or otherwise restrained, did not extend over the centerline of its support at least 6 inches:     (a) Ocean Medical Center, 425 Jack Martin Blvd., Brick, NJ: A Healy Long & Jevin, Inc. employee conducting concrete form work was exposed to fall hazards while working on a bracket scaffold.  The 16 foot long scaffold planks forming the platform were not cleated or secured and did not extend at least six inches over the center line of the end supports.  This caused a plank to dislodge and resulted in an employee falling 9.8 feet from the scaffold platform to the ground below.  Observed on or about 8/15/2013.      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
