29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  On or about 08/19/2013, the employer did not ensure that bales of textile materials were stacked or blocked in some manner to prevent tipping. The bales were stacked in 2 rows, 5 bales high, 35 bales per row, where the outer row, if struck by a powered industrial truck from behind, could topple over onto employees.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  On or about 08/19/2013, the employer permitted employees to operate the Catepillar fork lift without successfully completing the rtequired training. This exposed other employees in the area to the hazard of being struck by the lift when in operation.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by moving parts.  On or about 08/19/2013, the employer did not ensure that all guards were installed on the General Hydraulics Bailing Machine located in the main warehouse. The rear door of the Bailer had broken off and the sensor was tied in place to allow for the operation of the Bailer.

29 CFR 1910.1200(g)(1): The employer did not have a material safety data sheet for each hazardous chemical in use:  On or about 08/19/2013, the employer did not that copies of MSDS for all hazardous chemicals were maintained at the jobsite. The employer uses such chemicals as, but is not limited to, gasoline, diesel, hydraulic fluid, cleaning compounds. etc., exposing employees  to being unaware of the hazards associated with the various chemicals.
