29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about August 14, 2013, and times prior to, at the establishment located at 5027 North County Road 1015 in Blytheville, Arkansas, a Lockout/Tagout Program had not been developed for employees required to service and/or perform maintenance on equipment such as but not limited to the roll and brake press machines. Failure to do so exposed employees to caught-in hazards if the machines were unexpectedly energized.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: On or about August 14, 2013, and times prior to, at the establishment located at 5027 North County Road 1015 in Blytheville, Arkansas, employee wearing a 3M NIOSH half mask respirator while performing spray painting tasks had facial hair which prevented a proper seal. This condition exposed employee to health hazaards.
