29 CFR 1910.24(c): Fixed stairway(s) were not designed and/or constructed to carry a load of five times the normal live load anticipated or the strength to carry safely a moving load of 1000 pounds: On or about August 19, 2013, the employer did not enusre that the stairs leading to the rig floor were constructed to withstand a load of 1000 pound, two of the stairs had broken welds, exposing the employees to a stair failure and collaspe.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: On or about August 19, 2013, and at times prior thereto, the employer did not ensure that all of the alloy steel chains used on Rig #10 had identification tags that were legible and installed on the slings, exposing employees to being crushed by falling loads in the event of overloading the chain.
