29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location: Ralcorp, Inc: The employer failed to ensure that all electrical equipment operated in the area was approved for a DIVISION 1, CLASS II location. Employees were exposed to a fire/explosion hazard while cleaning silos and other locations where combustible dust was being utilized, handled and/or existed. A sample taken on August 13, 2013 was determined to be a CLASS II explosive material. a) Florescent lighting was utilized for overhead lighting in the flex line mixing area. b) Shop Vac vacuum: Dayton: Model #4TB84A was utilized to remove dust from the floor and other surfaces where dust accumulation was present.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area: a) Ralcorp, Inc: The employer failed to ensure that all exposed employees were provided training on the hazards of combustible dust. Employees were exposed to a fire/explosion hazard while performing duties involving the production of refrigerated dough and other bakery items. A sample taken on August 13, 2013 was determined to be CLASS II combustible dust. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
