29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:    a) Saw Mill  on or about August 19, 2013, wood chips and debris, such as wood blocks, trash and electrical cords were not cleaned frequently enough or removed to prevent slip, trip and fall hazards.       b) Post Mill  on or about August 19, 2013, wood chips and debris, such as wood blocks, trash and electrical cords were not cleaned frequently enough or removed to prevent slip, trip and fall hazards.       c) Maintenance Shop  on or about August 19, 2013, trashes, cardboard boxes and electrical cords were not cleaned frequently enough or removed to prevent slip, trip and fall hazards.       d) Post Mill  on or about August 19, 2013, the employers housekeeping practices did not include removing accumulations of fine wood combustible dust greater than 1/8 inch in depth from vertical and horizontal ledges, saw motors, electrical equipment and on the floor around the saw, which exposed employees to fire, combustion, and deflagration hazards.       ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED    Dupont Yard, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 314089798, citation number 1, item 001 and was affirmed as final order January 5, 2011, with respect to a workplace located at 103 Linnie St., Homerville, Georgia.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    a)  Sawmill  on or about August 19, 2013, numerous 480-volt control panel boxes had openings on the front cover from missing on/off switches, exposing employees to electrical shock hazards.      b)  Post Mill  on or about August 19, 2013, the 110-volt control panel box that ran the #6 post peeler had three openings on the front cover from missing switches, exposing employees to electrical shock hazards.        ABATEMENT CERTIFICATION IS NOT REQUIRED      Dupont Yard, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 310987425, citation number 1, item 015 and was affirmed as Final Order on March 18, 2008, with respect to a workplace located at 103 Landmark Rd., Homerville, Georgia; and in OSHA inspection number 314089798, citation number 2, item 002 and was affirmed as Final Order on January 5, 2011, with respect to a workplace located at 103 Linnie St., Homerville, Georgia.
