29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. 10 Nelson Street (Nelton Court), Hartford, Ct 06120: On or about 08/19/2013 an employee was walking and working on a second floor deck 12 feet above ground without utilizing any type of fall protection. The deck did not have guard rails installed.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: 10 Nelson Street (Nelton Court), Hartford, Ct 06120: On or about 08/19/2013 the employee was utilizing a fiberglass extension ladder to access a second floor deck and the ladder sides did not extend at least 3 feet above the deck landing surface. The employee was exposed to 12 foot fall hazard.
