29 CFR 1910.134(d)(3)(iv)(B): The employer did not provide an air-purifying respirator equipped with a filter certified by NIOSH under 30 CFR par 11 as a high efficiency particulate air (HEPA) filter, or an air-purifying respirator equipped with a filter certified for particulates by NIOSH under 42 CFR part 84 for protection against particulates: (Construction Reference 1926.103) (a) Benchmark Electrical Solutions, 302 3rd St. SE, Loveland, CO 80538: On and before August 19, 2013, the employer did not ensure that the cartridge on an employees 3M tight fitting respirator was equipped with a HEPA filter. The cartridge on the employees respirator was an organic vapor cartridge. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
