29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided. On or about 8/16/13, in the warehouse area, employees used electrical extension cords that had been improperly repaired as the outer sheath had been cut and repaired only with vinyl tape. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On or about 8/16/13, in the warehouse, employees were exposed to electrical shock hazards as the extension cord they were using had a strain relief that was exhausted.
