29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. #4 Turbine - On or about June 25, 2013, exposed parts of the exciter shaft less than seven feet from the floor were not protected by a stationary casing enclosing shaft completely or by a trough.

29 CFR 1910.269(p)(4)(iii): If, during operation of the mechanical equipment, the equipment could become energized, the operation shall also comply with at least one of paragraphs (p)(4)(iii)(A) through (p)(4)(iii)(C) of this section. #4 Turbine - On or about June 25, 2013, at least one of paragraphs (p)(4)(iii)(A) through (p)(4)(iii) (C) of this section was not complied with, when the 30 ton overhead crane was used to move a metal guard into position above the 100-250 volt exciter.
