29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) In the southwest corner of the plant a metal fourplex box was used as the end of an extension cord. The box contained knockouts and was improperly used as part of an extension cord. This use could result in an electrical shock hazard to employees. Abatement verification not required, the employer has provided evidence of abatement.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) On the west wall of the plant a 240 volt electrical box feeding a dry transformer, had unguarded holes from missing breakers and 2" diameter knockouts posing an electrical shock hazard to employees. The door to the box was open and accessible to employees. Abatement verification is not required for this item. The employer has provided evidence of abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) In the south center area of the shop, the power cord to a portable plasma cutter was not protected from abrasion of its power conductors due to the strain relief fitting coming loose. Employees were exposed to electrical shock hazards. Abatement verification is not required for this item. The employer has provided proof of abatement.
