29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard: (a) PRODUCTION AREA - Employees performed assembly operations where 110 volt fluorescent lights used for general illumination of the area were hung at a height ranging from approximately seventy (70) to eighty (80) inches, and the bulbs did not contain any type of guarding to protect the bulbs from accidental breakage, thereby exposing employees to possible electrical shock injuries, on or about August 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) HARLEY DAVIDSON HARNESS PRODUCTION CAROUSEL - Employees operated emergency stop and call for assistance control boxes where the strain relief for the cord entering the tops of the boxes had been compromised, exposing approximately one (1) inch of the energized conductors, thereby exposing employees to electrical shock injuries, on or about August 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
